- 100/- Per Copy if the Registration Number and Date of Registration of the Relevent Certificate in Known.
- If the Registration Number and Date of Registration of the Relevent Certificates is not Known, Rs: 200/- for the First Copy and Rs: 100/- for Each Additional Copy.

If it has been morethan 03 Months since the Birth in the Relevent Divisional Secretariat but it has not been Registered, the Following Documents can be Completed and Submitted to the Additional District Registar to Register the Birth
- Request Letter
- Completed Application
- Blank Request Document Stating that the Birth has not been Registered
- Marriage Certificate of the Child's Parents
- Birth Certificates of Older and Younger Children for the Child Concerned
- Written Evidence of Place of Birth ( Hospital Birth Report/ Gramaniladari Birth Report/ Estate Birth Report)
- Health Growth Chart
- School Admission Certificate/ Student Performance Index
- Other Documents Certifying the Date of Birth
- Gramaniladari Report
- Parent's Birth Certificates and Personal Identity Cards
- A Responsible Affidavit Declaring that the Date of Birth is Known
(Original and Certified Copies of all the above Certificates)

Registration of Birth Occurred at Home
1. Onform the Gramaniladari Within 07 Days of the Birth is Occurred.
2. Gramaniladari Shall send a Report to the Registrar of a Area.
3. The Birth is Registered by the Registrar of Births and Deaths of the Area Where Birth has Occurred.
4. Duly Filled Declaration of Birth shall submit to the Registrar of Births and Deaths of the Area.
5. Declaration of Birth can be Obtained from the Registrar.
6. Persons Required to give Information.

- A Person have in charge of the Child
7. Birth may be Registered by the Divisional Registrar within a Period of 03 Months. Birth can be Registered after 03 Months. Please see the Delayed Registration of Births.
8. A Certificate of Birth is Issued to the informant free of Charge.
